Order Form for Personalized Children's Books

We accept checks, money orders but prefer payments thru Paypal Email Invoice Service.

Include the following information when placing orders.

  1. Title of Book purchased.
  2. Name of Child: (Please verify spelling of name)*
    • First Name
    • Middle Name (optional)
    • Nickname (optional)
    • Last Name
  3. Age
  4. Boy or Girl
  5. City 
  6. State
  7. List of three friends or relatives
  8. This book is from:
  9. Special Note (optional)
  10. Payment Method
    • Check being mailed-Order will not be processed until check clears.***
    • Money order-Order will not be processed until money order received.***
    • PayPal Invoice-Emailed invoice will be sent once payment received order will be processed.

Additional Information needed for My Baby Book

  1. Date of Birth
  2. Time of Birth (include AM/PM)
  3. Baby's Weight
  4. Deliveried By
  5. Hospital
  6. Mom's First Name and Last Name if Different.
  7. Dad's First Name and Last Name if Different.
  8. Payment Method
    • Check being mailed-Order will not be processed until check clears.***
    • Money order-Order will not be processed until money order received.***
    • PayPal Invoice-Emailed invoice will be sent once payment received order will be processed.

Order Form for First Name Almanac

  1. Name (Please verify spelling of name)*
  2. Date of birth (optional)
  3. Personal Message (optional)
  4. Paper Selection (designer, white cardstock or beige cardstock)
  5. Ink color**
  6. Payment Method
    • Check mailed-Order will not be processed until check clears.***
    • Money order-Order will not be processed until money order received.***
    • PayPal Invoice-Emailed invoice will be sent, once payment received order will be processed.

Order Form for Bithday Chronicle

  1. Name (Please verify spelling of name)*
  2. Date of birth (optional)
  3. Personal Message (optional) or Headline of the Day
  4. Payment Method
    • Check mailed-Order will not be processed until check clears.***
    • Money order-Order will not be processed until money order received.***
    • PayPal Invoice-Emailed invoice will be sent, once payment received order will be processed.

* If name in product matches order form there will be no refund or exchange.  Exchanges will only be made if error is ours.  We will offer credit towards future purchase or replacement of same item.

**You can chose any ink color if using white or beige cardstock paper.  If using designer paper we will use best color selection that will compliment designer paper selection.
